You should receive an email from the Director of Athletics with instructions pertaining to all requirements for participation in athletics at Misericordia University. Below is a summary of the medical information required.
First-year or transfer students should receive an invitation to Misericordia's Emergency Medical Record software Healthy Roster. Please check your Misericordia email Quarantine folder for the email. Returning student-athletes already have an account.
Below is a list of necessary paperwork, which needs to be uploaded to Healthy Roser BEFORE arriving on campus.
1.A physical completed by a physician. Your physical must be completed within 6 months of the date of participation. You physical MUST have a box checked that states "CLEARED WITHOUT RESTRIICTION" to be accepted. Please use the provided form below.

4. Fill out the digital copy of The Emergency Contact and Insurance Information on Healthy Roster
